FAN INFORMATION:
- FBCS Warrior Athletics Facebook Group! You must request to join, and it is only open to our fans. If you are approved, an Administrator will add you to the group. This is also where we post game results, pictures, and other athletic news and updates. (Sorry, this is for our fans only – not available to fans from other schools.)
- Warrior Athletics Livestream Link through HUDL. Use this link for all FBCS Volleyball, Varsity Soccer and Basketball games. (This streaming site is accessible to anyone, not just our fans as it is no longer streamed on our Facebook page).

The online Google sports calendar will be the most up-to-date. (If you already subscribe to the full FBCS School Events calendar, you do not need to subscribe to the Sports Calendar in addition to that, as all sports info is also duplicated on the School Events calendar).
PARENT/ATHLETE INFORMATION:

Attention, Parents of Student Athletes: These items MUST be turned in before the FIRST GAME of the season.
We need the following items before the first game for an athlete’s participation in the sports program @ FOURTH:
1). Current Insurance Card (photocopy) – we need to collect this every year
We will need a copy of your student athlete’s medical insurance card (both front and back). Please submit the insurance card in one of these ways:
- Bring card to the office and we will copy for you
- Scan and email a copy of your insurance card to sfure@fourthbaptist.org OR sstephens@fourthbaptist.org
2). Sports Physical – MSHSL Sports Clearance 25-26 OR the 2025-2026-annual-health-questionaire (to be completed on the off-years when a sports physical isn’t required)
If your child plans to participate in sports at FBCS for the upcoming school year, he/she must have a Sports Physical done prior to participating. (Submit ONLY page 1 to the school office – keep the rest for your home records). Sports physicals are due every three years. Every year in between when sports physicals are due, the Annual Health Questionnaire must be completed and signed by both the parent and athlete. If you need a paper copy, you may print the PDF above or pick one up in the school office.
3). Immunization Updates – *Immunization updates are due to the FBCS office by the first day of school.
Please check with your child’s doctor to verify your student is up to date on vaccinations. See the updated Immunization Schedule for more information. If your child receives any new immunizations over the summer, please bring a copy of his/her immunization records to the school office before the start of the school year. Your clinic may also email immunization records to us at: sfure@fourthbaptist.org.
Part-Time (homeschool) Student Participation: Please see this info sheet – Part Time Student Info with MCAC form. The MCAC Homeschool Athletic Participation form on page 2 must be fully completed at the beginning of the season. Athletes wishing to play more than one sport only need to fill this out once.
